Liability Waiver

This release is entered into between the undersigned and Office Yoga® and its representatives and the undersigned hereby acknowledges that the following was explained to me and/or agrees to the following:

1. Acknowledges that the Office Yoga® instructors are not doctors or physicians and they are not trained in any way to provide medical diagnosis, medical treatment or any other type of medical advice.
2. Acknowledges that if the undersigned has been told that if they feel tired, feel pain or discomfort in any way related to the classes, that the undersigned should contact a medical physician immediately. That the undersigned has consulted with his/her medical physician and to the best of his/her knowledge is in no way prohibited from participating in a yoga class.
3. Acknowledges that any pose or physical activity carries with it the potential for damage or loss of property, serious injury and death. That the undersigned knowingly and voluntarily assumes the risk of participating in a yoga class or physical activity. The undersigned expressly waives, releases, discharges and agrees not to sue from any liability of personal injury, disability, death, loss of property or action of any kind the instructors for the undersigned participating in said yoga class or during physical activity.
